DOES THE VOLUNTEER NEED TO TAKE MEDICATION
If the volunteer has the need to take medication during program hours, the City’s preference is the have the volunteer self-administer the medication. If the volunteer is unable to self-administer the medication, or if there is the risk of an emergency circumstance where staff administration may be needed, please fill out the PARENTAL CONSENT TO ALLOW ADMINISTRATION OF MEDICATIONS form which will allow staff to administer medication to the volunteer. Please read the form carefully and follow all directions.
